（二）最近十二個月準時交貨的統計數據 Last 12 months on time delivery statistical data

（三）最近十二個月成品質量抽查的統計數據 Last 12 months final pre-shipment quality audit statistical data

1) 招聘守則 Guidelines for Recruitment

2) 職位說明書 Job Description

3)     廠規/員工手冊 Factory rule/ employee manual

4)     最新員工花名冊及身份證複印件 Updated name list and ID copies of staffs

（五）員工培訓 Employee Training

1) 員工守則培訓課程大綱 Factory Rules and Regulations for Employees 

2) 各個崗位特殊技能培訓課程大綱 Training contents of skills required for Individual post 

3) 員工培訓及考核紀錄 Records of Employee Training and Assessment

（六）質量系統的規範文件 Quality System Policies and Operating Procedures

1) 工廠組織架構圖 Factory Organization Chart

2) 質量主管的職責介定 Roles and Responsibilities of Key persons in Quality Organization

3) 質量主管的報告渠道 Reporting channel for persons in Quality Department

4)  質量控制程序及相關文件例如質量手冊，質量程序文件，各質檢崗位的工作指引，書面檢驗測試標準，抽樣計畫，工藝水平的檢查準則等 Quality control documents such as quality manual, quality procedures, operating procedures for individual quality control stations， documented inspection/testing criteria and sample plan and workmanship acceptance/rejection criteria etc.

5) 挑選供應商的手續及質量審查程序及相應審核記錄 Vendor selection procedures and quality assessment procedures and relative assessment reports

6)     合格供應商名單Accepted supplier list

7) 對供應商的質量要求Vendor quality requirements 

8)    供應商質量表現及監管紀錄 Vendors quality performance records and performance monitoring records

9) 來料及退料管理 Incoming material handling and non-conforming material return procedures

10)  各個檢驗崗位的檢驗記錄 (包括來料檢驗，過程檢驗及成品檢驗) Inspection records of each quality control stations (including IQC, IPQC and Final Audit)

11)  安全/可靠性測試報告Safety / Reliability Testing reports/records

12) 不良品處理程序及相應返修/複檢記錄Nonconforming material handling procedures and relative re-working/ re-inspection records

13) 不良品的成因分析及圖表及改善措施成效追蹤紀錄 Defect analysis records & charts and countermeasures monitoring

14)   貨物放行程序products release process.

15) 質量管理系統的年檢計劃及年檢報告 Master schedule for internal quality system audit, reports of periodical internal quality system audit

16) 按質量管理系統的年檢結果擬定的改善計劃及成效追蹤紀錄 Improvement plan against periodical internal quality system audit results and countermeasures monitoring  

17) 儀器校正計劃 Equipment Calibration Plan

18) 儀器校正紀錄 Equipment Calibration Records

19) 文件控制（含圖則、規格、包裝規格、產品安全規範文件, 同產品相關的標準/規範,如焊接，電鍍，UL標準等） Document control procedures (including drawings, specifications, packing requirements and product safety standards, relevant standards/specifications applicable to its type of business. e.g. Welding, Plating, UL standards, etc.)

20) 設備保養及維修程序 Machines preventive maintenance and repair procedures

21） 條碼掃描儀及條碼檢查程序及記錄Barcode scanners and barcode verification procedures and records

22) 倉庫管理程序及進出記錄Warehouse management procedures and in-out records

（七）消防安全政策和操作程序 Fire Safety Precaution Policies and Operating Procedures

 我们希望工厂维持一个安全的工作环境并且日常检查和监督以下方面 We expect the factory maintain a safe working environment with regular review and monitor on:

· 在适当的位置有足够的消防灭火设施、如灭火器、消防龙头、应急灯、出口指示标志和逃生线路图等等。这些都要符合中华人民共和国GBJ16-87和GBJ14标准 Sufficient fire fighting equipment are in place, e.g. fire extinguishers, water hydrants, emergency light and exit sign, evacuation diagram, etc. which meets PRC National Standard GBJ16-87 and GBJ140

· 保持所有的消防设施運作正常 All fire fighting equipment are under good working conditions

· 有足够的应急出口和保持通道暢通無阻 Sufficient emergency exits and unblocked passage

· 将易燃化学品（如油漆、稀释剂等）进行隔离和适当的储存 Segregation and proper storage of flammable chemicals, e.g. paints, thinner, etc.

（八）醫療設施及急救器材medical facilities and first aid equipments

l 設立適當的急救器材包括消毒劑，紅藥水，繃帶，燙傷藥膏等 Suitable first aid equipments in place include antiseptic, mercurochrome, bandage, burn cream, etc

l 急救人員培訓證 First air training certification

l 工廠醫師，護士，藥劑師,的相關資格證書（如適用）License of factory doctor , nurse and medical technician (if applicable)

（九）食堂執照及健康證明（如適用）Canteen License and Health Certificate of Cook (if applicable)
